Responsibilities:
- Provide primary care services to patients of all ages, including newborns, children, adolescents, adults, and seniors.
- Conduct comprehensive health assessments, diagnose medical conditions, and develop individualized treatment plans in collaboration with patients and their families.
- Perform routine physical exams, screenings, and immunizations to promote preventive healthcare and early detection of health issues.
- Manage acute and chronic medical conditions, including but not limited to diabetes, hypertension, asthma, and obesity, through evidence-based medical interventions.
- Coordinate care with specialists, allied health professionals, and community resources to ensure holistic and integrated healthcare for patients.
- Educate patients and families about health promotion, disease prevention, lifestyle modifications, and self-management strategies to improve health outcomes.
- Maintain accurate and up-to-date medical records, documentation, and billing codes in compliance with regulatory standards and best practices.
Qualifications:
- Medical degree (MD or DO) from an accredited medical school.
- Board certification or board eligibility in Family Medicine.
- Active and unrestricted medical license to practice in the state of California.
- Completion of residency training in Family Medicine from an accredited residency program.
- Strong clinical skills and knowledge of evidence-based medical practices.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust with patients from diverse backgrounds.
- Commitment to providing compassionate, culturally competent, and patient-centered care.
- Ability to work collaboratively in a team-based environment and contribute to a positive and supportive work culture.
